The framing problem
Semen analysis is filed in most men's minds under fertility, which means it is something you do when you are trying to conceive and never think about again. That framing costs you the more useful reading.
Spermatogenesis is metabolically expensive and sensitive to systemic conditions. It responds to sleep, to metabolic health, to inflammation, to heat, to alcohol, and to chronic stress load. That sensitivity is what makes it a poor thing to ignore and a good thing to measure.
What it is actually telling you
The parameters are downstream of the same systems that determine cardiovascular and metabolic trajectory. That is the reason the association exists between semen quality and broader health outcomes, and the reason it is worth reading as a systemic marker rather than a reproductive one.
The practical consequence for a senior man is that this is a signal that moves before the ones he is currently watching. Most executive health screening is built around markers that shift late. A marker sensitive to sleep debt and metabolic drift, on a production cycle measured in weeks rather than years, is telling you about the last few months of how you have been living.
Why it does not get measured
Two reasons, both non-clinical. The first is that the test sits in a fertility clinic, so a man who is not trying to conceive has no route to it and no reason to ask. The second is that it is embarrassing in a way an ECG is not, and senior men are not usually in environments where that is easy to raise.
Neither is a medical reason. Both are why the data does not exist for most of the men who would benefit from it.
What changes if you read it correctly
Read as a fertility test, the result is binary and only relevant for a few years. Read as a systemic marker, it becomes a repeatable measure that responds to what you actually change, on a timescale short enough to close the feedback loop.
That is the difference between a test you take once and a marker you can steer.
